I agree and acknowledge that I am freely and voluntarily participating in the Alpha Epsilon Pi events covered by this registration form and any associated services, programs or activities (collectively the "Event") and that I am traveling to and from the Event by personal or public transportation. I acknowledge and agree that my personal automobile and other personal insurance will serve as my sole source of recovery for any injury or loss I suffer, or as my sole defense for any claims made against me, arising out of or in connection with the use of my personal vehicle for Alpha Epsilon Pi related activities. I further agree and acknowledge that while the Event is being sponsored by Alpha Epsilon Pi, the Event is being held in rented facilities and certain services, programs and activities offered in connection with or as part of the Event may be provided on a contract basis by others. On behalf of myself and my legal representatives, heirs, family members, and assigns, I hereby release Alpha Epsilon Pi Fraternity, Inc., its directors, officers, employees and agents (collectively the "Fraternity"), from any and all claims, losses, liabilities and demands, known and unknown, for personal injury, property damage or other consequential, special or punitive damages of any kind, as a result of, relating to, based upon or arising from my travel to and from the Event, my attendance at and participation in the Event, the conduct of the Event or any act or omission with respect to the Event, and agree that I shall have no recourse with respect to any of the foregoing against the Fraternity.

I further agree and acknowledge that the Fraternity, and to those acting pursuant to its authority, has the right to photograph me and to record my actions and to use my picture and other reproductions of my physical likeness in connection with the Event or other Fraternity productions. All rights, title, and interest in and to all such filming and taping me shall be the sole property of the Fraternity, free from any claim by me or any other person.

An inherent risk of exposure to COVID-19 and other contagious diseases exists in any place where people are present.

By being on this premises you are acknowledging that an inherent risk of exposure to COVID-19 and other contagious diseases exists in any place where people are present and voluntarily assuming all risks related to exposure. According to the Centers for Disease Control and Prevention, senior citizens and persons with underlying medical conditions are especially vulnerable.

Health and safety measures have been taken for the safety of our guests, but each individual is responsible for use of the premises and personal conduct that promotes their own personal safety. All persons on this premises must follow all posted instructions and comply with all current national, state, and local laws, regulations, and guidelines. By being on this premises you agree not to hold the owner, lessor, operator, chapter, fraternity, guests, vendors, contractors, directors, officers, employees, agents, or volunteers liable for any illness or injury whatsoever.

All attendees will be required to show proof of being fully vaccinated against COVID-19, including a booster vaccination. All attendees, by registering for the convention, agree to wear a mask at all indoor events.

We are monitoring the information coming from the Center for Disease Control (CDC) and other government authorities regarding the Coronavirus (COVID-19) situation and will adjust all decisions as needed relating to compliance and safety.
